How it works

1. Activate Phyrexian Altar by sacrificing Cadaver Imp, adding .
2. Cadaver Imp dies, triggering Blight Mound and Liliana, Heretical Healer.
3. Resolve the Blight Mound trigger, creating a 1/1 Pest creature token.
4. Resolve the Liliana, Heretical Healer trigger, exiling it and returning it to the battlefield transformed, and creating a 2/2 Zombie creature token.
5. Activate Liliana, Defiant Necromancer's second loyalty ability by removing three loyalty counters from it.
6. Liliana, Defiant Necromancer is put into your graveyard as a state-based action due to having zero loyalty.
7. Resolve the Liliana, Defiant Necromancer ability, returning Cadaver Imp from your graveyard to the battlefield.
8. Cadaver Imp enters the battlefield, returning Liliana, Heretical Healer from your graveyard to your hand.
9. Activate Phyrexian Altar twice by sacrificing the Zombie and the Pest tokens, adding 1.
10. The Pest dies, causing you to gain 1 life.
11. Cast Liliana, Heretical Healer by paying 1.
12. Repeat.
