How it works
Cast Exhume by paying its mana cost
Holding priority, cast Dualcaster Mage by paying its mana cost.
When Dualcaster Mage enters, it triggers, targeting Exhume.
Holding priority, activate Altar of Dementia by sacrificing Dualcaster Mage.
Resolve the Dualcaster Mage trigger, copying Exhume
Resolve the Exhume copy, returning Dualcaster Mage from your graveyard to the battlefield.
Repeat from step 3.
This loop uses a clever recursive cycle centered around Exhume, which brings back whichever creature happens to be resting in your graveyard. By casting Dualcaster Mage to copy that very Exhume, you set up an endless chain where the freshly resolved spell pulls the Mage right back out of the dirt to do it all over again. Along the way, Altar of Dementia provides the necessary sacrifice outlet to bin the Mage each iteration, turning the endless cycle of life and death into an infinite mill engine that can grind through an opponent's entire library.
