How it works

1. Cast Fiend Hunter by paying its mana cost.
2. When Fiend Hunter enters, its first ability triggers, exiling Karmic Guide.
3. Activate Altar of Dementia by sacrificing Fiend Hunter.
4. When Fiend Hunter dies, its second ability triggers, returning Karmic Guide from exile to the battlefield.
5. When Karmic Guide enters, it triggers, returning Fiend Hunter from your graveyard to the battlefield.
6. When Fiend Hunter enters, its first ability triggers, exiling Karmic Guide.
7. Resolve the Altar of Dementia ability.
8. Repeat from step 3.

This elegant loop achieves infinite mill by leveraging the interplay between Karmic Guide and Fiend Hunter in a continuous cycle of resurrection and sacrifice. When Karmic Guide enters the battlefield, it pulls Fiend Hunter out of your graveyard, which in turn triggers its ability to exile Karmic Guide. By using Altar of Dementia to sacrifice Fiend Hunter mid-chain, you trigger its death ability to return Karmic Guide right back to the battlefield, restarting the cycle and allowing Altar of Dementia to mill your opponent out card by card.
