How it works

1. Activate Altar of Dementia by sacrificing Heartmender.
2. When Heartmender dies, its persist ability triggers, returning it from your graveyard to the battlefield without a -1/-1 counter on it due to Melira.
3. Resolve the Altar of Dementia ability
4. Repeat.

This looping masterpiece turns an ordinary insect into an endless mill engine by exploiting the interaction between persist and Melira, Sylvok Outcast. Because Melira prevents creatures you control from having -1/-1 counters placed on them, Heartmender's persist ability brings it right back to the battlefield completely clean every time it dies. You simply feed Heartmender to Altar of Dementia over and over, sacrificing it to force your opponents to mill cards equal to its power until their libraries completely vanish. While Heartmender's own upkeep trigger and persist mechanic usually require extra cleanup, Melira's static prevention completely bypasses the downside, leaving you with an unstoppable cycle of sacrifices.
