How it works
Activate Altar of Dementia by sacrificing Hogaak.
When Hogaak dies, The Skullspore Nexus and Ratadrabik of Urborg trigger, creating two creature tokens.
Resolve the Altar of Dementia ability, causing you to mill eight cards.
Cast Hogaak from your graveyard by exiling five cards from your graveyard and tapping two black and/or green creatures you control.
Repeat.
This loop turns Hogaak, Arisen Necropolis into a self-sustaining sacrifice engine fueled by the graveyard's resources and convoke. Whenever Hogaak is fed to Altar of Dementia, Ratadrabik of Urborg catches the legendary death to spawn a nonlegendary zombie copy, while The Skullspore Nexus simultaneously prints a massive Fungus Dinosaur token based on Hogaak's power. Those fresh tokens (alongside any other creatures lying around) pay for Hogaak's convoke cost, and exiling the cards you just milled feeds its delve ability right back from the graveyard. The result is an unstoppable cycle that mills out your library and buries your opponents in infinite triggers.
