How it works
Activate Faeburrow Elder by tapping it, adding at least .
Activate Altar of Dementia by sacrificing Derevi.
Choose to return Derevi from your graveyard to the command zone.
Resolve the Altar of Dementia ability, causing target player to mill cards equal to Derevi's power.
Activate Derevi by paying 1, putting it from the command zone onto the battlefield.
When Derevi enters the battlefield, it triggers twice.
Resolve the first trigger, untapping Faeburrow Elder.
Activate Faeburrow Elder by tapping it, adding at least .
Resolve the second Derevi trigger, untapping Faeburrow Elder.

Finally, Altar of Dementia acts as the grim engine that feeds Derevi back into the command zone while continuously milling out your opponents with every sacrifice.
