Every time Defiant Greatmaw re-enters, its own triggered ability places two -1/-1 counters on itself, which immediately annihilates the +1/+1 counter thanks to state-based actions—clearing the slate so Mikaeus, the Unhallowed can catch it all over again on the next sacrifice.
