The magic truly happens because Vizier of Remedies alters the math on Heartmender's persist ability, ensuring it returns to the battlefield completely free of the -1/-1 counters that would normally shut the loop down. Because Heartmender comes back clean every single time, you can sacrifice it over and over again to feed Thermopod's mana-generating maw for an endless cascade of enters-the-battlefield, leaves-the-battlefield, and death triggers.
